If the customer has a valid Manitoba driver's licence or identification card, they need to provide their document number to confirm their identity

The form needs my ID number, which i found easily, and it also needs my document number. As per the DMV site, it should be on the lower right of my card by where it says I'm an organ donor. The only thing in that corner is the organ donor heart and words, no number.
